Based on your completed intake form, Community Glow will personalize a Notion care hub that may include:

The $99 setup includes one personalized care hub using Community Glow’s standard Notion system. After your intake form is received, your completed hub will be delivered electronically within seven business days, along with instructions for duplicating and using it.

This service organizes information supplied by the family. Community Glow does not access medical records, provide medical advice, monitor care, interpret treatment instructions, or replace licensed healthcare professionals.

What the Standard Setup Includes

The standard $99 setup includes customization of the existing Community Glow template, up to five family or caregiver contacts, and the care-management sections relevant to your household. Extensive customization or additional databases may require a separate quote.

How It Works

After checkout, you will receive a detailed intake form. Community Glow will review your information, personalize the hub, and provide access within seven business days after receiving the completed intake form.

Before You Purchase

A free Notion account and a valid email address are required. The purchaser is responsible for deciding who receives access and for protecting sensitive information entered into the hub. This purchase does not include ongoing hub management or updates by Community Glow.
